What you need to know about CATCH-H (Coordinated Access To Care from Hospital for the Homeless)?

CATCH is a collaboration between Unity Health Toronto- St. Michael’s Hospital & St. Joseph’s Health Centre, the Centre for Addiction and Mental Health, LOFT Community Services, Inner City Health Associates and Good Shepherd Ministries. CATCH is an integrated team of case managers (provided by LOFT) supported by primary care and specialized mental health providers for people who experience chronic or recurrent homelessness and are not adequately connected to health services in the community.

CATCH works from a critical time intervention approach to meet individuals in the hospital while they are accessing services and, in the community, to support transition and bridge resources into the community.  CATCH works with individuals and referral resources and provides assessment and engagement to individuals who are experiencing homelessness who are accessing emergency departments and inpatient units in the City of Toronto. CATCH provides individuals with client centered interventions to assist in connecting to needed health, mental health, substance use and housing resources in the community.  The aim of the CATCH Program is to provide a low barrier transitional case management service that meets individuals where they are, supports access to medical care, and improves access and connections to resources in the community.
