The Homelessness Health Services Mapping Tool details clinical services, including primary care and psychiatry, and harm reduction services available at sheltering and drop-in sites across the Toronto region. This Mapping Tool helps to reflect what is currently in place, where there may be gaps, and opportunities to create connection to services where needed. It is also meant for providers and site staff to connect clients to services as transitions occur across sites. It is currently managed and maintained by ICHA and Parkdale Queen West Community Health Centre.

On Board Referral Pathway
On Board is a central referral and intake system (CRIS) that was created to connect non-insured, unattached people living in the city of Toronto to Community Health Centers where they can access comprehensive ongoing primary care. On Board services will be extended to support the sheltering and drop-in systems. It is intended for site staff (not client self-referral) to assist in connecting clients to CHCs for continuity of care. It is currently operated by Access Alliance (AA) in connection with CHCs, FHTs, ICHA and other primary care providers.
