Publically Funded Physiotherapy Clinics

Individuals are eligible if:

• referred by a physician, and
• covered by OHIP, and
• under 19 years old / over 65 years old, orrecently discharged from hospital and in need or physiotherapy that directly relates to the condition for which the person was admitted to hospital

Or:

• referred by a physician, and
• have physiotherapy coverage through OW or ODSP

Parkdale Community Health Centre

Individuals are eligible if:

• they live in the PCHC’s catchment area (map available here)
• they bring a referral form if not an active client at PCHC
