Street Health ID Clinics

Street Health’s Access to Health Cards for the Homeless program helps individuals through the process of obtaining a health card, including obtaining the other identification that is required before getting a health card.  Many of the people they help come with no identification at all.

Service Ontario

Service Ontario can replace health cards that are lost or stolen. Individuals must provide one original document that proves Canadian citizenship or OHIP-eligible immigration status, one original document that proves residency in Ontario and one original document that displays a name and signature.

Service Canada

Service Canada can replace a SIN number by ‘confirming’ it. Patients must apply in person and present original documentation, such as a birth certificate or permanent resident card. There is no fee. Information on how to apply can be found here, and on required documentation here

Permanent Residency

Permanent resident cards are only issued or replaced via mail. Applicants must submit a photocopy of a passport or travel document, as well as a photocopy of their record of landing document, confirmation of permanent residence, driver’s license, photo-identity card or an income tax assessment, as well as other income tax assessments. Two photos are also required, as well as payment of a $50 fee.
