Required Hours: 4-8 Hours / Week (0.1-0.2 FTE), preferably on a Thursday.

Description: Streets to Homes (S2H) and partners provide street outreach and housing-related follow-up supports to assist people who are experiencing homelessness and sleeping outdoors in finding and keeping housing. This includes outreach to individuals living in encampments. The ICHA clinic at SHARC/129 Peter St. is in a large well-lit office immediately adjacent to the office of the counselor who serves as the dedicated health liaison; the psychiatrist will be located onsite and will not be doing street outreach. A security officer is stationed just outside or in their cars. 129 Peter St. serves some of the city’s most vulnerable homeless, including some who are barred from previous shelters and come to 129 Peter for showers/laundry/food. Streets to Homes counselors accompany some patients to appointments.

Description: Yonge Street Mission- Davis Centre is a drop in Program. They have a team of Psychotherapists that the ICHA Psychiatrist works with to support clients. The mental health professionals provide support for emotional difficulties such as anxiety, depression, social isolation, relationship conflict, grief, addictions, and more. Using a trauma-informed approach, our interventions build resilience and strengthen psychological wellbeing for Torontonians experiencing poverty, in person or virtually, free of charge.

Description: 4117 Lawrence Shelter program opened in May 2021. The program can serve up to 89 beds for those experiencing homelessness in Toronto. It serves all genders. This program is an emergency shelter program that provides temporary accommodation and related support services that assist residents to move into housing. The provided wrap-around services include: daily meals, laundry, recreational programming and case management supports.
CONC – Comfort Inn – 445 Rexdale Blvd

Description: The Hotel Shelter at 445 Rexdale Blvd. provides overnight shelter supporting the well-being of 160 clients each night in Toronto’s North-West End who are homeless, street-oriented and facing mental health challenges, substance use issues, unemployment and other barriers to maintaining adequate housing.
Comfort inn also has the Breaking Barriers programming, which is accessible to all program participants and shelter residents of CONC. Breaking Barriers case managers work closely with their clients to formulate individually tailored case plans, helping them set short and long-term goals to stabilize the client and develop the skills, tools and resources necessary for them to thrive.
