Study Title: Cardiovascular Care Experiences of People Experiencing Homelessness and the Co-Design of a New Care Management Program: A Multi-Method Qualitative Study
Dear colleagues,
I hope you are well. I am currently conducting a research study on understanding the care experiences of people experiencing homelessness and heart disease, eventually to co-design a new heart disease management program in Toronto.
I am interested in interviewing various multi-disciplinary care providers about their experiences and challenges providing heart disease management for this population and your insight on what would make a new program effective. This can include cardiologists, family physicians, internists, nurses, nurse practitioners, social workers, case managers, cardiac rehabilitation specialists, outreach workers, among others.
Your perspective will be very helpful. The online consent form and demographic survey will take approx. 10 minutes, followed by a 30-45 minute interview over Zoom. You will be compensated with a $20 coffee gift card as a token of our appreciation.
