Who we are and what we do
Street Clinical Outreach for Unsheltered Torontonians (SCOUT) provides episodic and transitional healthcare for unsheltered people experiencing homelessness directly where they are living (e.g. encampments, ravines, under bridges, sidewalks).
Our mobile team is comprised of two nurses, a case manager and a family doctor. We provide clinical care that is low barrier, patient-centered, trauma-informed, and harm reduction focused. Common issues that we assess and treat include skin and soft tissue infections, cold and wet weather injuries, mental health concerns, addictions, sexual health, chronic health conditions, and musculoskeletal issues. We also connect clients to primary care and assist with system navigation, as appropriate.
We service all of Toronto.

Expect to receive a phone call from one of our nurse’s within 1-2 business days to obtain further information.
Note that if your client is experiencing an emergency, it is best to bring them to the nearest Emergency Department.
