Regional Mobile Nursing

ICHA’s Regional Mobile Nursing services provide comprehensive transitional medical care to individuals accessing ICHA services at partnering shelters, shelter hotels, supportive housing programs, and drop-ins. The regional mobile team is regionalized into North, East, Downtown-East Regions, with each region supported by primary and substance care Nurse Practitioners, Registered Nurses and Registered Practical Nurses. The primary care and substance use care nursing teams provide care in hub sites as well as through mobile and virtual care channels, and integrate fully with ICHA’s primary care and specialty physician teams to deliver robust clinical supports covering the continuum of healthcare needs across Toronto.

Referrals can be made via our secure online form and are triaged by our Virtual Nurse Coordinators upon submission. The referral form can be accessed via: referral form. Examples of common referrals we receive include wound care, STI testing and treatment, immunizations, bridging prescriptions, non–urgent medical needs that require attention prior to the next on-site established physician or other provider clinic day, and coordinating care and referrals with external community health providers.

Our interdisciplinary team works closely with internal programs and external partners. These collaborations assist patients with their health & psychosocial needs and encompass psychiatry services, Indigenous health programs, hospital-community transitions, addiction services, specialized medical care, community resources, and connections to case management and peer support. Additionally, the team supports individuals with health system and social service navigation which allow for continuity of care and transition to other community primary or substance care providers when appropriate and possible.

This will enable us to process the referral smoothly and ensure that all necessary information is captured.
