Multi-Disciplinary Access To Care & Housing (MATCH) provides ongoing intensive support to individuals referred from the Multi-Disciplinary Outreach Team (M-DOT) and Coordinated Access to Care for the Homeless (CATCH) teams who require the ongoing involvement of an interdisciplinary team.

The MATCH team consists of Case Managers, a Registered Nurse (RN) and a part-time ICHA Psychiatrist providing ongoing care and support for clients referred through M-DOT, CATCH or The Access Point.
