PRIME

As of June 2026, both Regional Mobile Nursing and Population Health Services have merged to be the PRIME Program (Primary Care & Population Health in Integrated Mobile Engagement).  This program provides care in the form of; Primary Care and Population Health services.

PRIME (Primary Care)

PRIME Nurses provide transitional primary care across shelters, shelter hotels, supportive housing, and drop-ins.

Care is delivered across North, East, Downtown, and Mid-West regions of City Of Toronto.

Interdisciplinary teams include:

  • Nurse Practitioner and Virtual Nurse Practitioners
  • Registered Nurses
  • Registered Practical Nurses
  • Community Health Workers

The nurses deliver care in designated clinic spaces provided by the hosts.

Clients are seen on appointment basis, with a referral submitted to the team beforehand. The referral can be submitted by; shelter/drop-in staff & case workers, ICHA clinicians, and Hospital discharge planners (Social Workers, Nurses, Outreach worker etc).

Services provided by PRIME nurses include:

  • Wound Care
  • STI Testing & Treatment
  • Immunizations
  • Bridging Prescriptions
  • Non-urgent medical needs between physician/NP clinic days at our partnered sites.
  • Care coordination and external referrals to community resources.
  • CHC/FHT/FMD navigation

Eligibility for PRIME services; Individuals who are homeless and/or precariously housed whether insured or uninsured regardless of their immigration status.

Referrals can be sent from the following settings:

    • Shelters and shelter hotels
    • Transitional housing
    • Drop-ins
    • Hospitals

Kindly expect to be contacted by one of our nurses within 1-2 business days to obtain further information for appointment planning.

Note that if your client is experiencing an emergency, it is best to bring them to the nearest Emergency Department.

Services are available regionally (refer to the Locations tab on the website) based on program service capacity.

 

PRIME (Population Health and Mobile Engagement)

The Population Health focus of our PRIME team involves: Vaccination Support where we set up clinics to provide immunization to our homeless population, and Health Promotion, where our team empowers individuals with lived experience of homelessness to make informed decisions about their health and well-being.

The interdisciplinary teams include:

  • Community Health Workers (CHWs): Who provide frontline outreach, and navigation to community supports. Their work also includes distributing harm reduction supplies, safer-use kits, facilitating sexual health promotion and vaccination education sessions.
  • Registered Nurses/Registered Practical Nurses: Who administer seasonal vaccines (Flu, RSV, Covid-19, and Pneumonia) and primary care vaccines (Hep A/B, Shingles, and HPV) at the clinics we set up at our partnered sites.
