Primary Care and Medical Specialties
Overview
The Primary Care and Medical Specialties program (PCMS) provides accessible, comprehensive primary healthcare for people experiencing homelessness, delivered directly in shelters, drop-ins, and respite settings to reduce barriers to accessing care. PCMS clinicians play a vital role in providing medical and social care to people experiencing homelessness across more than 60 sites and programs, serving diverse populations-including youth, seniors, and refugees among others-across a wide geographic area. PCMS assigns clinicians to sites to provide transitional, community-based primary care on-premise, with clinics operating as appointment-based, drop-in, or blended models depending on clients’ needs. The program offers full-scope primary care, including chronic disease management, assessment and treatment of acute and complex medical and social conditions, as well as preventive care.
We are committed to walking alongside clients on their complex health journeys, supporting their transition out of the shelter system and facilitating connections to longer-term community primary care such as Community Health Centres (CHCs) or Family Health Teams (FHTs) including the Inner City Family Health Team, while emphasizing continuity, interdisciplinary collaboration, and equity-focused care for individuals facing significant social and structural barriers.
