Meet the Population Health Team

The Population Health team works to strengthen community well-being through coordinated care and innovative health initiatives. Led by the Director of Population Health Services, the team brings together a diverse group of professionals, including a Program Manager, Program Coordinator, Health Promotion Coordinator, Physicians, Registered Nurses, Registered Practical Nurses, Community Health Workers and Health Assessors.
