Who We Are and What We Do

The MATCH program provides ongoing intensive supports to individuals referred from the Multi-Disciplinary Outreach Team (M-DOT) and Coordinated Access to Care for the Homeless (CATCH) teams who require the ongoing involvement of an interdisciplinary team to support their success in the community.

The MATCH team consists of Case Managers, Peer Support Workers, Registered Nurses (RN) and part-time ICHA Psychiatrists providing ongoing care and support for clients referred through M-DOT, CATCH or The Access Point.

MATCH HFRESH (Housing First For Encampments and Street Homelessness)

MATCH HFRESH is an expansion of the MATCH model. The project is looking to evaluate a new housing and support program for people in encampments and other unsheltered locations who have moderate or high needs for support.
