M-DOT (Multidisciplinary Outreach Team)

The Multi-Disciplinary Outreach Team (M-DOT) is an interdisciplinary team of providers that deliver services to vulnerable individuals on the street and in shelters.

Who We Are and What We Do

The Multi-Disciplinary Outreach Team (M-DOT) is a program funded through the City of Toronto, Streets to Homes, and works in partnership with LOFT Community Services, Inner City Health Associates (ICHA) and the Centre for Addiction and Mental Health (CAMH). The team is made up of Outreach Mental Health Case Managers, a Peer Support Worker, Registered Nurses, and part-time ICHA Psychiatrists who connect with individuals.

M-DOT provides clinical hands-on support to those with complex healthcare needs, who are experiencing chronic homelessness. Services are designed to be short-term, focused on comprehensive assessment, stabilization, and linking to long-term resources. The program provides intensive transitional outreach and clinical services to individuals who are experiencing chronic street homelessness, street involved or staying in emergency shelter and who present with complex physical health, mental health and/ or concurrent disorders. M-DOT referrals are deemed appropriate by the team when all other models of services have not effectively been able to support successful health and housing outcomes for each individual. The aim of M-DOT is to provide low barrier, transitional, intensive outreach and clinical supports that meet individuals where they are at and assist in stabilizing their physical health, mental health and substance use.  M-DOT works from a housing first and harm reduction approach. The goal is to meet immediate needs, reduce barriers, and assist individuals in getting connected to housing, and follow up support that meets the level of need required for each individual to live successfully in the community.
